Advisory Committee Appointment & Graduate Degree Program Form

Graduate Faculty Nomination Form: Please complete this form if you wish to nominate someone for Graduate Faculty status to serve on an advisory committee. Submit the completed form along with a current copy of the nominee's CV to the Graduate Program Assistant. The Graduate Program Assistant can also confirm an individual's current Graduate Faculty status if you are unsure whether a nomination is necessary.

SES Exam Information Forms: Use these forms to start the process of scheduling an exam. Please complete the appropriate form and submit to the Graduate Program Assistant at least 6 weeks prior to suggested exam dates.

University Exam Request Form: Please submit to the Graduate Program Assistant at least 4 weeks prior to the exam, with Advisory Committee and student signatures on page 2.

Doctoral Final Examination Arrangements: Please submit this to the Graduate Program Assistant, along with the University Exam Request form, for doctoral final exams.

Forms & Resources for Students

Graduate Course Waiver (add/drop form): Required for Special Topics courses (obtain the course instructor's signature first, sign it yourself, then submit to Graduate Program Assistant) and for late registration (sign and submit to Graduate Program Assistant).
